How do I access RevolutionPHR?

Click on the button below. Your username is your first initial of your first name, first initial of your middle name, last name and last two digits of your birth year with no spaces. (Ex. John M. Smith born in 1978 would have a login of jmsmith78)

What is RevolutionPHR?

The acronym PHR stands for “Personal Health Record” and represents an online location where you and only you can view important information about your relationship with us.

RevolutionPHR utilizes the highest standards of online security while allowing you to view portions of your eye care record through any standard Internet browser on a PC or Mac computer or an iPad We encourage you to use the Login information created specifically for you to review the many features available.

You can decide if access to RevolutionPHR is right for you, and participation is completely optional. We have no record of your access and we allow you to use this online portal at your discretion.

What can I do in RevolutionPHR?

  • Update us with changes in key information such as new phone numbers, email addresses, addresses, and more.
  • Complete your pre-exam interviews online. This information automatically is added to your patient chart in our system which leads to a more accurate and efficient exam.
  • Schedule appointments any time of day. (This feature will be available soon.)
  • Check the status of glasses and contact lens orders.
  • See billing statements.

What to Expect

When you visit our office, you will be warmly welcomed and we can answer any questions you may have about completing the paperwork so that Grand Lake Eyecare may see you. Our team will work together to evaluate your eyes and provide relevant eye care treatment information, review your vision benefits and payment options, fit you for glasses or contact lenses, and focused on exceeding all of your expectations. If at any time you have questions about the process or your specific health information, please ask.

Payment Options

Grand Lake Eyecare believes in providing the best value for you and your family. We have products and pricing that aims to accommodate all budgets and our office participates in most insurance programs. To learn more about our payment options, please call our office directly.

Our office accepts cash, check, and major debit and credit cards including MasterCard and Visa, as well as Care Credit.

Don’t forget to use your:

  • Health Savings Accounts (HSA)
  • Medical Savings Accounts (MSA)

All of the above plans can be used for all your eye care and eyewear needs, including eyeglasses, contact lenses, routine office visits and medical office visits.

Care Credit

Insurance

Since there are so many plans and coverage varies widely, please call us to find out if your plan covers care in our office. Our staff will verify and explain your benefits before your visit at no charge.

Infant-See:
A public health program where Dr. Gerlach, Dr. Dues, and Dr. Evans donate their time to provide vision assesments to children under 1 year of age.

A non-profit organization that supports research and education about keratoconus.

State funded program that helps provide eyecare education to our local schools.

Provides accurate, up-to-date information on macular degeneration.
